Likely a fuel air issue. SO the afm could be failing or there could be an issue with the fuel delivery.

Sounds like faulty ECU to me. Have you checked it?

Default 94 ls

This is what I've done for my fuel system....all preventive maintenance. Change spark plugs, wires, distributor, fuel filter, pcv valve..... run a fuel system cleaner thru it (SEA FOAM).... if you need a new CPU try finding one online and install yourself.... save a TON OF MONEY!

Hook up a vacuum gauge and see if reading drops at steady higher RPM

I don't know if the 94 LS400 is OBDII compliant but if not, at least on older Toyotas, jumpering TE1 and E1 on the diagnostic socket revealed the engine diagnostic 'blink codes' that correspond to the issue.
