Question Removing a rear axle

Been a long time since I've been on here. GF is taking too much time away from my car and interweb time . Anyway my car has been making a clunk sound going into reverse and drive from the rear. It also makes this noise when letting off of the gas or right when I accelerate. I was under it checking to see if something is loose. Found that the driver's side axle boot by the diff. is pretty much gone. So I am assuming this is what is causing the LOUD thunk sound.

Has anybody had to replace an axle on one of these? From what I have found so far it does not seem very difficult but some advice if possible would be helpful.

What tool is needed to remove the lock cap and nut. in the center of rotor?

1992 LS400.

so the hex bolts are pretty freakin stuck. Called the dealer for labor only they say $300. local indie that I trust said it would only be about 80 bucks to install.
