All in all the ISX50s are pretty solid cars with minimal issues...but with any used vehicle, I would highly recommend having the car looked over by a mechanic before purchasing. Could be a number of things creating the noise...
Definitely get the car checked out by your mechanic, even though lwxus and toyota hold a great track record on reliability, you never know what/how the previous owner may have treated the car.

Check if it has had all the recalls done.
My car had the rattle noise on cold startups when I first got it but an oil change helped a bit. Then when I got the recalls done, it has been quiet ever since (about a year).

Take it to Lexus and see if they will do the recall on it. If so, buy the car and then have Lexus replace a bunch of parts while they have the engine down like the timing chain, spark plugs, ect.... You will basically have a new engine on the cheap!
