Hey there, I recently got a 2001 IS300 with unknown miles, as the guy replaced the cluster and I had not noticed until my rev suddenly stopped reading. That isn't my issue, though. I noticed the day after I got it that I don't have a first gear, and consequently I can't use manual mode or take off too well from a complete stop. Is this a sign of a new trans? Or something more simple? I don't want to get rid of this car because I love everything about it but I do want to fix that if I want this car to stay in my life.

Take it to a transmission shop. Get the fluid flushed really good. Could be as simple a grunge in the tranny. Otherwise, I'm sure the shop can tell you what is needed.
