We always use premium since it's required for the hybrid. We now have 148k miles and all is still good. Knock on wood. We also always use Costco gas and keep up with all required maintenance at the dealer.

I bought new injectors from mylparts.com (they give 10% discount to clublexus owners) and had my local mechanic replace them. New part number on injectors BTW. You will save prob $1000 cm going to stealership. Runs like new

I've barely had the car a little over a month and its a Certified Pre owned so I will let Lexus get the parts from whom ever as they will be responsible for it. Kinda scares me as this is the second time I have had to take it back, 1st time was a short in the rear tail light, now this problem and also has developed a rattle in the glove box trim around the lock. Yikes hope this does not continue! Thanks!!!

I've just found out that the 2GR-FXE engine employs EGR - exhaust gas recirculation.

Perhaps this explains the cruddy buildup on the injectors!

Does anyone know how to do an EGR delete on these engines without causing the check engine light to come on?

Found some pictures of how the intake manifold has plumbing to introduce exhaust gases back into inlet manifold here:

I'm having the same issue with my 2016 RX-450h with 30,000 miles. It's at the Lexus dealer in Escondido, ca. currently. I received a call yesterday from the SA saying it has 4 injectors that are foiled. We took it in last monday and he said it will not be ready until next monday. Your description was exactly what was happening to our RX-450h.

Did they offer any insight or explanation into why this has happened to your vehicle, which is relatively young of age?

Being a 2016, you have a 2GR FSE engine which is a newer design to the 2GR FXE employed in the MY15 and earlier model years of the RX450h.

My 2013 RX450h is with the dealer for the exact same problem! It has about 35,000 miles, always 93 octane gas from BP or Mobil, and oil and filter change every 3500 miles. Just can't believe it!

This winter the rattling noise on cold start reared its ugly head again. I guess the injectors are dirty again.

By chance I found a workaround. When the air-conditioner is set to AUTO with the temperature above 16 degrees Celsius then the engine starts without any rattles.

Yeah, cuz when you have it on Auto ICE runs basically quite much more than not on Auto. It has to maintain hot coolant and guess what - on hybrids, ICE runs quite less, so it cools down.
As the result, you have consistent oil pressure in the system, that is reducing rattles. Not injectors. I bet you rattle is timing chain tensioner, that happens to be hydraulic oil based, so it backs off and allows chain slap.

My 2012 RX450h was displaying some very slight erratic stumble when first started in the morning, and the automatic choke is on, which elevates the idle revs slightly.

It only has 31,000 miles on the odometer despite its age.

I bought a can of LiquiMoly Injector Cleaner, put it into the fuel tank and topped up the tank.


During the course of that tank, I would drive normally but whenever it was safe to do so on the highway, I would fully depress the accelerator pedal to the floor, to really rev out the engine.

I've not noticed the symptoms anymore after the treatment. Coincidence? Maybe. Not sure if the cleaner has helped, but it may be worth your while to run thru some injector cleaner through the tank of fuel perhaps once in a while to keep the injectors clean. I bought the most expensive cleaner I could find in the auto store, but I would imagine most brands all do the same thing.

That's my terminology for the fast idle when the engine is started from stone cold, as I don't know what Toyota/Lexus calls it.

What do you all think? Do you think injector issues are common with engines with EGR systems? Should we build in a fuel injector cleaning as part of our regular maintenance from now on? Every 10K miles or 15K miles?

The last time I had injector problems was the mid 90s on a GM car. I currently have 3 cars in the driveway with a collective total of over 500,000km between them. None have ever had injector cleaning, none have ever had any engine performance issues.
