Only once, but it wasn't just out of nowhere. Another car was drifting towards my lane, I was in Autopilot and it over-reacted. It's never happened other than that one time in almost 8K miles

It happened to me a few times during autopilot, and it was the car in the next lane suddenly drifted toward my lane each time. In one of the incidents, the Y braked so hard that it caused the car behind to fishtail when he braked so hard. That was a close call.

That didn't happen, although if someone was tailgating me it could have been a problem. I think Tesla has addressed many of the phantom braking issues
