iPhone 4S iPod Issues
I listen to podcasts and many times when my phone connects to the media I am unable to select type of music/podcasts/genres/artist/album. The indicator is blank and the pointer wont even go to it. I have to make my selections on the phone. I have tried disconnecting and reconnecting media devise but t no avail.
I am getting frustrated with many of the media/nav functions because they are not intuitive and don't play well together. I have started a list of things that frustrate me and may list them here soon. Some have been addressed in other threads but there are small nuances that could have been (should have been) designed better.

I suggest going to your dealer's trained Lexus Technology Specialist to assist you with this issue.

I did notice this problem, the workaround I do which works is o back one time and come agin the same spot and press the mouse and it then comes to the next screen with the playlist,albums,genre etc. first time that screen is blank.

Are you paired to BlueTooth Audio along with handsfree phone functions? And using the sync cable plugged into the USB port to access your podcasts? I ask because the term "iPod" only appears on the media page when an Apple device is physically plugged into the vehicle. Conflicts arise when the same device is used as two audio sources.
If that is the case you'll need to choose one or the other method to hear your files through the car sound system.
USB cable will charge your device but needs to be plugged in each time you enter the vehicle.
BT-A will play when phone is in jacket pocket or hip holster but uses battery power.
Ensure you have the most recent Apple updates for your phone and do a hard reset of the phone after any updates to the iOS.
If that is the case you'll need to choose one or the other method to hear your files through the car sound system.

I have done both, bluetooth access, which looses the ability to control whats playing because the selection buttons don't work. When someone says a "work around" it just reinforces poor integration in the Lexus design (don't blame Apple).
I have accessed it with the USB cable as an iPod which gives you the browse button and access,BUT now having issues with selecting the podcast I want, it plays, but the displayed information did not updated to whats currently playing. Workaround-turn the car off and on.
I love my car but hating the design of the electronics, not intuitive and the navigation sucks. Access to functions and wrong directions, wrong exits labels, AND yesterday it had me exiting at a service plaza to get to my destination. ALL OTHER GPS never did that. (google maps, apple maps, Waze, Mercedes Navigation)

When you use as a plugged in iPod you have BlueTooth turned off on the phone???
You have downloaded the latest iOS???

2. Plugged I tried using an iPod Touch 4th Generation
3. Running 6.1.1 (will upgrade to 6.1.2 but that is a bug fix for exchange server)

Or stream your music via BT-A but do not plug in the USB cable. Does your missing podcast reside on the phone? On an iCloud? It is available to you with all the information the car doesn't display when you plug in earbuds?

The issue could actually be with iOS 6.. A ton of people have been complaining about how the bluetooth drivers were updated in iOS6 and broke compatibility with a ton of 3rd party hardware. I have an ES as a loaner and my iPhone 4S has been great (though short amount of time). I'm running 5.1.1.
As a reference point, I have a iPad 4 with 6.1.1 and it was connected to my bluetooth in my IS (3rd party setup) and I was getting a ton of random skips during a road trip. With my iPhone 4S everything runs smooth as butter. This is just with basic streaming too, but I've heard from a friend how his buddys headunit won't display music info anymore after updating.
