Dreyfus, My drivers side lamp just went out on me and i'm really glad that i found this thread. The dealer wants 175 for a bulb, but i'll get two off amazon for $100 and replace them myself with your instructions Thanks a bunch, err $175 (a replacement bulb plus dealer labor cost above me doing it myself)

Thanks everyone for your comments about replacing the headlights. I read the thread and was able to replace my front passenger headlight on my ‘07 ES350 with 177k miles. The original headlight was a Harison Japan D4S 35W bulb (Toshiba brand) which I replaced with a Philips D4S 35W 4300k bulb that I purchased on Amazon for $45. I took it to a local mechanic and he changed the bulbs for me for $5. The dealer was asking $175 per bulb and another $80 for labor. I appreciate all of the insight and helpful information provided on this topic. Thanks again!

Yes, there is a reason behind people calling dealers - "$tealers".
Example: I had a suspension repair on one car I owned, where the lowest price I found was around $1,200.
Bought the best quality parts on Rock Auto for $180 (low-end units were under $85), and an Independent installed for another $150.

One of the HIDs on my '07 ES350 went out in 2014, replaced both with Phillips bulbs from Amazon at $90/pair, no problems in the 5 years since then.
BTW... aftermarket ballasts (effectively, transformers) are available if *that* turns out to be the problem.
Currently (07/19) amazon offers a plug-n-play ballast together with the D4S bulb for about $51
