It sounds like something is seized or binding and the caliper is not sliding as far as it needs to. Those are the right pads, and they're the right thickness. Once the piston is fully retracted and the caliper can slide properly there is more than enough room for pads of that size to fit in the caliper.

IS200d/220d/250/300/350 pads are all the same size for the rear.

So the dealership was able to install new pads, not sure what the problem was. I requested they use my pads; however they did not (why would they right?). ~$80 for pads and with labor and rotor resurfacing came to be a whopping $315. I hate the dealership so much.

I have 06' IS350 and planning to replace my rear brake pads next week. When i check online where i normally order my Toyota parts i found 2 parts number (see below). Comparing below I believe the correct part number is 04466-22190, and I called Tustin Lexus now and they confirmed 04466-22190 is correct. So I guess your pads is for IS250 only?
  • Manufacturer:Lexus
  • Part Number:04466-53010
  • Part:Brake Pads
  • Part Notes:IS250. 2wd, 11.4 rotor. Without sport susp.

Default rear calipers

yeah I just run into this issue on my IS350, driver side caliper this time. It won't slide outward enough to get the new brake new outside brake pad in between the rotor and caliper. Passenger side has a new caliper so it slides everywhere, pads were installed easy-peasy.
I guess I could try to heat that pivot point. The caliper will rock back to the rear of the car, but it is very stiff and you have to hit it hard with you hand to move it backwards.
annoyed.
I just put the old pads back on. I almost put a new pad on the inside next to the piston, plenty of room there, but I didn't. So one side has new pads, one side does not, and has the 60K pads. and at this point, I don't give a crap.

The front brakes calipers are so nice compared to these dam things on the rear.
I'm looking for new rear calipers
grrrrrrrrrrrrrrrr
