Default Car slight hesitation when braking

Hi, anyone experience when you are braking when the car is about 5mph or under. The car like hesitates ( seems like it going to stop, then goes forward again ). The issue is not over bearing, but you definitely can tell it doesnt stop real smooth when coming to a complete stop.
Another way of explaining it is, it seems like the brakes grab, then for a split second they let go, and grab back on the rotor, makes the car seem like its slightly kinda jerking forward.
Another thing i noticed is when i have my foot on the brake at a light or whenver i have to have it on for at least a few minutes, sometimes the brake goes softer all the way down and the rotor/brake makes a noise. The brake seems like its giving up and the car rolls forward just slighty even with my foot still on the brake all the way pushed down. I just have to let go and reapply the brake again. Im sure both of these may have something to do with each other. I got my brakes resurfaced about 9 months ago along with new brake pads. any suggestions? could it be air in the brake fluid or something.

Thanks

I would say your brakes are:

1: fading (over heated) [grabs and lets go]
2: warped (from over heating)

How clean is your brake fluid? Should be a light honey/golden hue. If anything other than this, probably time for a flush.

What brake pads did you end up going with?
