I initially thought that the clamp was just loose (like the screw/bolt had given out or that it had worked its way off the pipe from vibration/heat), but once I started trying to loosen the bolt...

Culprit is...

a cracked exhaust clamp. Passenger side aft of the resonator. I pulled it off the car tonight. It was rubbing against the heat shield as it expanded and contracted.

Amazon and Advance Auto all say that that Motorcraft FL-400S does not fit the 2001-2006 LS430. Can anyone confirm these websites are not accurate?

For everyone's benefit - $322 for new rotors, pads, and wear sensors. Figure $4/ caliper bolt (you need 8 for the whole front and rear job).
