According to OBD readout your O2 sensor heater circuit not working. Could be a bad O2 sensor, or the connector/harness, OR the fuse. I don't have RX this week but there should be fuses for the O2 sensor circuits. Otherwise, let the dealer handle it. If it's a simple fuse, you can change it but if it pops again then something shorting out

i have been to the dealer, all they did was clear it and tell me to keep an eye on it. and now that it has come back i am calling and getting them to come pick it up and give me a loaner because i have very limited free time to mess around with them since they are 45 minutes away

Wow!, did you hit a hole or something at the same time? that is unusual for it to bend or misaligned something just from a panic stop. or did you turn the wheels at the same time as the panic stop?

Nah it was all clear on the road. the calipers on the breaks ended up shifting somehow. so Lexus ordered the strip and re-assembly. everything is fixed.

About your "Check AWD message" and "TRAC off", I've seen them before on a 2015 RX - they are caused by some OBD readers, usually when you leave the car on for several minutes without starting the engine. Unplugging the reader will fix it.

You did not say exactly what the dealer do to fixed the problem. From the receipt, it looks like they change some gasket in the exhaust manifold, is that the one that fixed the problem?
At any rate glad that it is fixed and hopefully the code does not come back.

I am a Lexus master tech. Odds are that you simply got a bad cat from the factory. 93 octane would do nothing do cause this as the A/F and O2 sensors can adjust to any changes in fuel in a fraction of a second. I have not seen this problem personally, so I would hope that it is just an isolated problem.
