A few years ago while having my Lexus serviced I asked the service writer how important it was to used premium fuel (required in my '09 ES350). I was told not to use regular or mid grade AND to only use Chevron. From what I recall they said there are additives in Chevron gas that prevent engine sludge.

Poor quality gasoline/weak additives will not cause engine sludge. It may cause carbon build-up, but not engine sludge. They are two completely different things!

Top tier gas now appears to be at all Costco gas stations:

Definitely some savings in my area ($.14/gallon) over the other likely options. Here is another description of their program.

Ironically, it seems to indicate that their prior gas product would have lead to carbon deposit build-up when compared to top tier gas. This was the reason that I avoided it in the past.

Well i guess that explains why the gas at costco near me was like 40 cents cheaper then the Shell like 1/4 mile away. I just fill up at my dads gas station. I need to get me a costco card to shop. Im not happy with Sam's Club anymore.

Not necessarily. Top Tier gasoline is above all a marketing effort (it's on their website) so just because a gas isn't a Top Tier doesn't mean it is inferior.

Was the same left over Fast Eddies gas you get at Vons now, and if you buy enough groceries you can get ANOTHER 50Cents off a gallon on that crap.......
