04 Wiper blade refill part #?

Any local auto part supplier can help you. There are probably three different brands out there. get a quality set from pep boys or autozone and you will be happy. If you really want the "oem" part number call your local Lexus dealer or go to Sewell parts online, a CL vendor and they have a parts lookup by year and model for your Lexus.

Not sure why you want the refill. Its much easier to replace the whole blade, just snap out and snap a new one in. I use Bosch blades, which cost between 12 and 25 dollars, depending on how fancy you want. All of them have performed extremely well for me. Autozone, Pep Boys etc have them in stock, or you can get them online.
The rear blade is another story. As far as I know, you must use a refill, since no one sells a replacement other than Lexus. I buy the refills from Toyota, they are around 5 bucks.

Use the single blade wipers. They rock. All my cars have them now.
Bought a Bosch set for the RX, it is sitting waiting for my OEM refills to wear out...6 more months!

I'll assume the part numbers for the '05 are the same. If you end up wanting the blades as well, the part numbers are 85212-0E010 and 85222-0E010 ($33.20 and $39.60 CAD...quite pricey). I recently bought these new blades, but haven't been impressed since they produce too much noise at the point the blades switch direction. When I bought the vehicle it had Goodyear blades and they were quiet but simply wore out. I prefer OEM but in this case should have bought Goodyear blades again.

I have been using the Bosch Icon blades for several years now and find them exceptional (these may be the "single blade wipers" noted above; the ones that "rock"). They look great, they last longer than any other blades I have ever had, and most of all, work very well. They are a little pricey, but not twice as much, even though they last twice as long.
Highly recommended.
