Push and hold the screen button until you hear a beep.

To be specific to what ggebhart said: let's say you are on preset #7. Tune manually to the station in AM or FM you want (or scroll manually to the XM station you want) then press and hold either remote side button to enter it into preset #7. You can follow this procedure to enter stations in the rest of the presets. Here's an example; if you want 107.7FM to be in preset #2 instead of the 102.9FM that is in there now, just scroll to preset #2, then manually tune to 107.7FM, then press/hold either remote side button till it beeps and 107.7 will then be in preset #2

Does page 158 in the Nav manual help you understand this a bit better?

How do you delete a preset after it has been set?

You don't delete but you can overwrite. You can also choose how
many pages of six (6) stations you want between 1 and 6 for a total
of 6 to 36 stations. If you cluster all your genres together in a page
such as AM and FM and XM news stations in one and any Sports in
the second and then Music together. When you run out of ideas then
go to Settings (on the AM and FM pages only, not XM) and cut down
the excess pages.
Use the small rocker switch between Map and Menu (Home & Menu
on 2016's) to jump between pages then use Mode on the left steering
wheel to select within a page. Don't forget that a long press of the Mode
button is a Mute/Pause such as when Nav directions are being drowned
out by the radio.

They are identical to the ones included with the vehicle but having them in electronic form makes it much easier to find things with a simple search.

If you sign up for Lexus drivers you can have access to all the ones that belong to your vehicle.
