Auto trans won't shift to highest gear

Hey everyone, I'm new to the forum (and auto trannies) and have a transmission question. 95 SC300 with 90K miles and it won't upshift to the final gear. Any ideas? I've already flushed the tranny fluid.

It lights up on the dash, but won't shift past third gear I believe. Basically stuck at about 40 mph max

I can hit the od switch on the shifter to illuminate O/D OFF and then then again to put OD back on, but it still won't upshift.
