The Lexus Soarers Club of Austrailia (sry, no link at the moment) has a product (from Japan) listed in their sales area that uses an extremely hot chemical of some kind placed in a can of water (to prevent burned carpet) that is "set off" like a bug bomb with the air system running to remove ALL mold, odors etc from the air system completely process takes about 15-20 minutes and "supposedly" removes ALL types and kinds of odors. Worth a shot if all else fails. Additionally, autopia (link avail here somewhere) has a baking soda process described in their forum that is used by pro detailers to remove odors..
