Installed another throttle body. Same thing.

Adjusted the kickdown cable numerous times to numerous positions.
Same thing.
What in the world could it be?

Another thing I've noticed. Occasionally when I start the car with the engine cold it shifts normally until I stop the car and restart it after it has already warmed up, like when I go to a gas station. After I restart the car it shifts up to Overdrive immediately. Maybe it's related to engine temperature then.

Here are my current engine codes after installing and adjusting the new TPS properly on the newer throttle body:

21 - Main oxygen sensor signal (on left bank)
25 - Air-fuel ratio lean malfunction
41 - Throttle position sensor signal

There are no transmission codes.

Today I removed the throttle body again, cleaned it entirely out, checked the adjustment screws, checked if everything was in place, tested the TPS using the Ohm method, reset the ECU, and cleaned out the IACV.

Nothing has changed. The idle is still at around 2000rpm, there seems to be a minor misfire when I take off from a stop, and it shifts into O/D too fast.

I'm beginning to suspect that the ECU decided to take a crap.

Check the post in the LS400 forum about " All my crazy electrical issues gone! Leaking ECU capacitors" I have had success on all three of my Lexus vehicles with the leaking ecu capacitors. Check it out, follow the directions exactly and you will be fine. It's a super cheap repair that almost always cures these weird issues. I'm talking like 4 dollars in parts from the correct supplier. LsCowboyLs and Japanese ecu engineer Yamae are the ones who found this information.
