Default Grinding when braking and accelerating?

Hey fellas, I've got a 1998 LS400 and have a question. The car has newer brakes on it. When you are moving in reverse and apply pressure to the brakes they grind, then when you put it in drive they feel like they hang up/grind for about 30 feet then the car is fine. When you accelerate quickly there is a bad grind and when you go up a hill the car grinds very badly. The car only has about 75,000 miles on it and has had an easy life. Any ideas to what this grind is?

Maybe a seized brake caliper?

it is not a squeal. it is a true grind sounds like metal on metal like when a pad is grinding into a caliper. the pads are all good, none of the rotors have grind marks, i have taken everything apart and put it back together twice now and greased all slide pins and see nothing unusual on any of the brakes. Do these transmissions have grinding problems?! it grinds badly when going up hills starting off, and under semi and hard acceleration (worse when vehicle has been running for long periods of time). cruising on the expressway smooth, driving around town smooth, shifts well, drives good. just grinds badly at the points i have stated.

have you checked the trans fluid, rear end fluid, and the trans/engine/diff mounts?
