I would first take a look at the cheap part...02 Sensor, it reads exhaust gas and tell the computer what to do. How did you pull the code?

1990-1994 LS400's are OBD-I, so no code reader is necessary. Codes can be read by shorting pins TE1 and E1 in the DLC2 connector, turning the key to 'ON' in the ignition, and counting the number of time the MIL flashes. A short pause between flashes indicates that the MIL is showing the 2nd digit of the code. For example, *FLASH* *FLASH* [PAUSE] *FLASH* *FLASH* *FLASH* would indicate code 23. There will be a longer pause after the code sequence and the code will repeat until you turn the key back to the off position.

That is the same as standard toyota, I thought this would still be OBD-1, Very nice, just for information is there a good site for the code definitions?
Thank you.

Did you replace all 4, or narrow it down to one?
