I was having similar issue with brakes clicking/Jerking noise during low speed.Eg in parking lot or when going over speed bumps.I took it to dealer and they replaced the front both assembly.Said there was TSB -SU1219.Waiting to see if the problem goes away.I will post update as soon as I have one.

Does "slap" occur when the vehicle is stopped? The acid test for a bad actuator for me was pumping the brakes while stopped (never letting off so much that the vehicle could move). The crunching, popping, squeaking noises screamed "Actuator!" and completely disappeared when the bad one was replaced. Good luck.

I have 2010 LS460 and been driving Lexus since 1992. Break noise is not normal on the top of the line Lexus. If anyone from dealer tells you because they don't know TSB and didn't follow instruction on TSB, I been struggling with Lexus dealers and head quarter to convince them that if noise comes from the top of the line car and not from cheaper models, it's design problem in Lexus. After several months going back to few dealers, one dealer told me that the brake actuator was replaced but didn't help. I think they didn't do anything and just told me that they did. Can't trust some dealer, some are very dishonest. After I took my TSB to the dealer I trust, they used TSB and fixed my noise.

Had the 2010 back to the dealer to see if they would change the accuator because of the TSB , but the one on the car was the updated version. They did go thru the trouble of checking all the lines, rotors , pads and calipers. So I guess the noise I hear when braking is kinda normal. I am still trying to find ls 460 drivers in my area that may experience the same problem or try to get a ride in their car to hear if there is any noise. Just doesn't seem right for a car that cost that much should have brake noise.

Brake noise Lexus LS 460 2012
I think that this may be my same problem. When braking at low speeds like at a stop sign or when cold (below 40 F ) I get noise in the brakes. Not sure which brake but I can hear from inside the car. Is this the same problem, requiring the same fix.
