Default CSV & check engine light

Ok, this has been driving me a little crazy lately, but here is the issue I'm having or my wife is having really when driving our 1998 LS400 (118K miles)
When the car is in motion its fine, when it comes to a stop, the check engine light comes on and then cuts out, then the CSV light comes on too. I have to turn the car off and turn it back on for it to go. If the car already has the light on it does the same thing when i come to a stop. Also, at times when I am driving the car I press the gas and it does nothing, I continue to tap the gas until the car moves. It almost seems that there is something in the fuel line.....This happened to my Honda once, but the reason for that was the the gas tank had water in it. Any ideas would be great!

first replace fuel filter. if no offect diagnose and replace fuel pump.

Thanks. I'm gonna probably end up flushing the fuel line, replacing the filter, and see where that gets me.
