Jeff, maybe you can confirm this.
I was told that for Lexus Canada from the local dealership there that, the TSB for Canada because they cant refer to the ones which is for US.

USA TSB's are not valid in Canada.
Canadian TSB's are not valid in USA.
There hasn't been a TSB released for the headlight bubbling issue in Canada.
If you order a new headlight for a 2011, you get a pair of the newest 2012 headlights from Lexus.
These can still be replaced in Canada before the TSB is released (if it is), but they would just do it as a normal warranty issue, not a TSB.

I initially informed my dealer of the bubbling issue when I first learned about it. They made a note under my account stating that there was definite visible imperfections but said nothing can be done unless Lexus acknowledges and is willing to pay for new headlights.
Skip a few months later and now the USA has issued a TSB. Recently, as my car was getting serviced, I informed the service manager of the USA TSB. Just like Jeff has said, the TSB's are different from US to Canada and at the time said they will have to wait until Canada releases one, but since one was so bad they were going to replace just the drivers side. Skip a few days later and I received a phone call saying that they just ordered me both headlights and will call me to book an appointment when the new ones arrive! Again, just like Jeff has mentioned, because there is no official TSB out in Canada, they are doing the work as warranty work. Hope this helps other Canadians out.

When I went to my dealership for an oil change before the TSB existed, I just told them I had a paint defect in my headlights and that I wanted them to look at it. If they determine it is covered under warranty, you should not have to pay anything at all.
