As long as it is an official Lexus disk it doesn't matter where you buy it. There are threads galore on how to do the update (search is your friend); the disk is behind the clock so you have to remove that to eject it. It's a ten minute operation at most to do the swap.

The clock panel was difficult for me to learn to remove at first. I now use the supplied tools and some clear plastic packing tape, strategically placed, to pull the panel from the dash. Regarding where you buy the disc, it doesn't matter but it needs to be the same "generation" NAV unit as the one installed in your car. Dealer will charge you $200 or so for the disc and $45 for the labor to install.

search for similar threads. I just did it and I suck at doing anything on my own. You will find the 2 pointed tools needed in the wheel lock tool pouch. When you push those pins in the radio face, do it one at a time. If will give you beter control. And you do push it all the way (the thin pencil part). When you get one side in, do the other. Once both sides are done, the face will come out. The Nav Setting will given you the option to eject disk. When the new one goes in, it will take a while to download. Once that is done, it is easy to put the face plate back in.

Oh! I do use the Nav alot and it was a good upgrade for me. 2 niggles I have noted. 1) it likes the long way around when there is a short route. 2) I like the split screen with the right side being turn by turn direction. It sometime switches out of the mode and goes to a different style of split screen.

No real argument on that point, but you may want to play around with the settings on the route preferences screen to see if you can find a combination that works better for you.
