Failed smog with Fsport intake

Failed smog with Fsport intake
Anyone gone through the same? I failed smog with Fsport intake due to missing sticker. How do I go about getting a new one ? Appreciate any input.

The last time I went through smog testing, I didn't have the sticker so I just showed them this form:

and they just passed me. not sure if it will work for you though, but you can try it if you want.
I keep this in my car to, just in case a cop decides to be a douch.

visual fail, hopefully they gave you a free until you pass option.
I had a visual fail on my 1995 4 runner because a vacuum hose to the intake broke off when the guy touched it (it was brittle as hell), he wanted to charge me a second test fee when I brought it back, I was mad as hell because he broke it. He ended up calling his manager at home who authorized 1 free retest.
be careful what facility you chose.
