Oh really? I thought the cam recall was for the 350 and the TSB is for the 250.

And when I log onto the Lexus website it says I have no open recalls for my vin

I've taken it in previously about this noise and they came back and told me I needed to leave it there for a piston ring recall. They did a re-ring and exhaust midpipe free of charge, and water pump just parts waiving the labor fee. They took care of me in this situation, however now the can problem I have previously complained about is still a problem. I have opened a case in corporate for disputing the diagnostic fee, and I may call back and extend it to the cam problem.
I along with many other owners shouldn't have to pay for an engineering flaw.

You are correct that there is no recall for the cam gear noise on the IS250.

Only a TSB exists...

Not necessarily. If you call National, 1-800-25-LEXUS, you may be able to get them to goodwill, or cover a portion of the repair.

I didn't read that far back in the thread, but have you had your 250 rebuilt for the campaign already?

So any update? I'm really annoyed by this noise that happens during the startup... It's so nerve-wrecking to hear a Lexus making this noise.

I have a 08 and been having this problem for years. I have like 160k miles. The dealer won’t do this service for me for free will they?

No, they won't. Cost is approx $2500 to have it done on your dime.

I've had this intermittently on my 2006 IS250 AWD since I got it in 2008. Recently it’s become more frequent. I’m over 160,000 miles so I doubt it’ll be covered under any warranty at this point.

What’s the outcome when the part fully fails? Is it something I can wait on to fail completely and then get fixed or will it break other things on its way out?

I switched to a thinner oil and it fixed my issue. Originally using 5W-30 to 0W-30. I had that slight rattle on cold startup about 4 years ago. After I switched, haven’t heard it since.

The noise will get louder and louder and won't be intermittent. You'll know when (if) you need to get it repaired long before the gears fail completely
