This happened to my car as well. They had my car in the shop for 3 days. I believe there's an issue with the cam gears amongst other parts.

I have the rattle in around 1 in 4 startups. I've found that it happens most often with "hot" starts. I took it to the dealer last month and they could not reproduce it so they did nothing. I may go by there again after running the car around town for a while to heat the engine oil in hopes that the sound presents.

My concern is whether the engine is being harmed by not fixing the "problem". As long as it's intermittent, I guess I'm at the mercy of it showing up when a tech is within earshot .

The first time I let them know about this, they (Sewell Lexus) kept the car for one day and could not reproduce the rattle noise. I had to let them know the noise is not only during cold starts. This noise usually occured after the car has been "warmed up". I dropped off again the following week, and the noise was duplicated after a few days of keeping the vehicle. After one month... no more embarrasing noise and starts like it should. There is a lot of work done when this is repaired. Minor surgery.

2 Gear Assy Camshaft
Gasket timing chain
Gasket Camshaft bearing
Insulator Fuel Pump
2 Gasket Cylinder Head
Gasket Air Surge TA
FIPG Oil Pan

The "noise" doesn't bother me, per se... I'm just worried that the rattle is a sign of an underlying issue (and based on your parts list it must be) that may eventually harm the engine or lead it to seize unexpectedly one evening while I'm driving in the middle of nowhere. No idea if my concerns are justified. Any gearheads have a comment?

I have that problem too especially last week it was doing it on almost every morning/night starts. Right now it's being good. I just went to the dealer and they could not reproduce the sound. My warranty is running out (@49k right now) and I am worried that if this causes a problem later on, it might not be covered under warranty? Anyone know if this is a big problem?

Don't know if it's a big problem, but the dealer in Austin fixed mine, same day as the 3 recall letter things. It's listed on the repair order as:
RATTLE NOISE PRESENT FROM VALV TRAIN ON START UP CAUSED BY VVTI GEARS
TSIB#L-SB-0156-08 R&R INTAKE CAM GEARS
OFP#13050-31##

The parts list was very similar to the one by DZ... above.
Sounds like your dealer is stalling, or something. I'm sure they all know about it, by now.

My 1st 08 had this issue. The new 08 doesnt.

Also, dont forget your car is direct injection so its gonna be noisy regardless. My ex-gf's Audi A4 2.0T sounded like a disel at idle. lol
