just use a flat head and pry the whole plastic cover out. it's brutal force, and pretty scary, but it will come out. do NOT just take out the lens, it's together with the cover, you will crack it.

reading bulb / map light
My sister has an 02 Sportcross. It took me a while to figure this one out. Be careful with the "brute force" advice. Like I said, I worked on this thing for a while and I tried yanking on it; it felt totally wrong. I then found that with a trim removal tool I was able to open it. If you put a trim removal tool/ wide screw driver in too far (deep) and pull down on the whole housing, that felt wrong and it felt like I was going to rip the entire housing out. At least on the Sportcross with a moonroof, the trick was to get between the plastic molding edge and the rest of the housing with switches. The plastic rectangular molding and the light cover popped down leaving the sunroof swithces in place. I had to get the bulb at the dealership, since the same shaped bulbs at the auto parts places where much lower wattage. The bulb in my sisters car was 10 watts. I even looked on the Toyota technical website, but the diagram was not detailed.
