There is no light on the dash. The car run fine no jerking after stop if I easy on the brake. The issue is pronounce when I make a moderate stop. Also, the issue start after the engine warmed up.

That's sad but true...the envelope is always being pushed...cars need to be lighter but still perform...something has to give. I plan to keep this car for as long as possible...I don't drive it too much anymore but I'm still going to be preventive when it comes to maintenance...when they give a lifetime warranty on the transmission i'll believe in the lifetime fluid...

I haven't changed my tranny fluid but i know my car jerks a bit in general when stopping... does the pedal go all the way to the floor? that could be your rear caliper seizing... or maybe if it doesnt but it goes down lower than usual could be front pads are wearing out... rotors are warped....

It's now resolved. I took the car back to the dealer yesterday. They found that the tranny fluid is about 0.6 quart low. After they fill it up to to proper level, all symptom went away. Back to smoothness.
I still have to call them back on Monday to fix my paperwork and report in lexus owner website since they fat fingered on my paperwork. My car currently have 59k on it. Some one fat fingered put down 89k. And it showed up in lexus owner report. (2 days prior that they reported that I went in for 60k service at 59k miles on my car. How can a person put 30k miles on their car in 2 days???).

i'm glad to hear your issue has been resolved....but it's sad that the Lexus service dept botched it from the beginning. I mean....when the LEXUS SERVICE DEPT is the reason for the PROBLEM...it's a shame. It's just another reason I personally do as MUCH of the maintenance on my cars as I can...because I know I'll put great care into each and every job. Some hung-over service tech may or may not give crap if they "do it exactly right" on someone else's car.
A few months back my brother needs an oil change. Dealership has a decent wait so he hits a Jiffy-Lube. Basic oil change here...nothing more needed. Guy comes to the waiting room and says, "got you all fixed up and ready sir". Bro pays the money and exits the other side of the garage in his Acura. He doesn't get 30 feet before he hears a horrible knocking from the motor. Alarmed as hell he simply pulls back around to the front and drives right back into the open bay. The morons had forgotten to put new oil BACK INTO THE CAR. I mean...really??! How does one literally fail on HALF of the job?? Scary.

After I got the car back, I felt that the shift is back to smoothness...even smoother than it was before.......
I too do not believe in lifetime fluid... It would be a lifetime fluid if you plan to keep the car for a few years... I generally keep my car for a long time and I do all preventative maintenance on all my cars.... My 03 Honda accord is my commuter and it has 180K+ Miles on it and still drive like it was a new car.
I will be taking my 2011 RX to get the tranny service soon... This time I told my SA to make sure that the Lexus diagnostic specialist will be involve to make sure that they put the fluid back at the proper level....
