My moms car ended up just having a bad #5 injector, all fixed and running 100% again.
That's good to hear that this thread helped someone else also.
My problem will be fixed once I get another injector. I just haven't had time in the past few days... living in Iowa and dealing with these relentless snowstorms hasn't given me much motivation to get this RWD car back on the road. I'll just keep driving my truck for now.
I forgot to update this in case somebody else could use this info in the future. I replaced the suspect injector and now my car runs like a champ. No more rough shakey idle!
I ended up getting an injector from rockauto.com whom distributes GB Remanufacturing. GB reconditions OEM fuel injectors. I could've bought a brand new injector off rockauto.com's site but they were aftermarket brands. I wanted the EXACT factory Toyota issued DENSO injector. For only $29 plus shipping I got a OEM Denso reconditioned injector with the exact flow rates and specs as spec'd by Toyota. In the near future I will send all 8 of my matching injectors off to get cleaned, balanced/blueprinted.
There's a couple places that'll do this. They'll ultrasonically clean, preliminary flow test, clean again, test again until all of your injectors are flowing at the same rate. Plus they will outfit your injector with new pintle caps, o-rings and filter baskets. It costs roughly $16-$17 per injector.

i had a pretty bad misfire problem with my 99 maxima a while back...while it's a very different engine it ended up that oil was somehow making its way into the spark plug shaft (the VQ30DE is a coil-on-plug setup), as well as the plug-to-coil connector boot seemed to be cracked and so the spark pulse was arcing against the wall of the shaft before it got to the plug, causing a serious misfire. We ended up just ripping apart basically the whole top end, cleaning everything, resealing it and putting it back. Took care of the oil part but the 6 connector boots (little rubber pieces) would have cost me almost $300, and the remaining, very sporadic misfire was not such that it was costing me extra fuel or even happening very often. In retrospect i would have gone at the injectors and head gasket next, as the car had 145k.
dunno if that maybe sparks a new path for you; my recommendation would be to just go back over everything, top to bottom as if you had a fine tooth comb, just look for every tiny stupid little thing that could throw the engine off. These engines are pretty sophisticated and very fussy, the slightest error will cause huge issues. haha the new lexus commercial about cylinder tolerances with the dust is not an exaggeration.
