Default Transmission jumps from a start...low idle problem?

This has been happening to me for a while now and i've never really thought anything of it, but figured i'd ask just to be sure ( and peace of mind)

I'd say about 50% of the time this happens. Ill be coming to a stop from normal speeds, and ill be at about 5mph then ill go to hit the gas and the car will lunge forward. I've also noticed while pulling into my long driveway, and im slowing down to around 5 mph and the car will downshift hard...well hard for a lexus . I've noticed the idles at about 500rpm when this happens so im thinking that's whats causing it, but any suggestions? thanks for any insight

We have a curve incline, and if we dont come in with enough speed, the transmission down shifts and the car lunges. I attribute that to going into 1st gear and the boost of toque you get. On flat road, the vehicle surges when you expect it (stomp on the pedal )
