From what I've seen, every model that uses Ni-MH cells has the same 6-cell modules, with the exceptions being the ls600h and the rx400/450h.
Simply swapping the "bad" cells isn't the best way to do this - read up. Lots of info on priuschat and similar sites, same procedures apply.
