i have the 3G iphone, paired with a 2006 GS3, and have similar issues. I too can't hear the rings when dialing out.
I have another related question/issue. When I dial out from my phone while in the car, I get 3 audio options displayed on the phone: use the phone as the source, use speakerphone on the phone, or use the car audio via bluetooth. When I select the phone option, so that i use the phone to speak into and hear from, the bluetooth in the car is engaged, and displays the bluetooth screen, muting the radio/CD.
My question is, if I selected the source of the audio to be my phone, why does the car still activate the bluetooth screen on the car? Its annoying cause the radio/Cd switches off while the call is being made.
is that a lexus issue, or a iphone issue?

i think it's a quirk with the iphone because of the oneclick source choice you describe. the stereo just knows it's linked and can tell the phone is in use so it mutes the music because it has a simple brain: phone in use, voice on the way, kill sound. the fact that you choose a different 'output' for the sound rather than the car is an iphone thing. I doubt that apple or lexus will agree it's an 'issue'.
my 3g works the same way with my GS. doesnt bother me, i'd turn down the sound anyway even to make a call via the phone. personal preference for me.

I am curious how the rep transfered contacts from your iPhone to your Lexus? I have an original iPhone with an 07 GS450h and was told that the phone can't do bluetooth transfer of contacts, which is what the Lexus is looking for.

I have an iphone 3G and had no trouble pairing it to my 06 GS. on the iphone, settings, general there is a screen to turn on bluetooth, and the hands free shows up there when i go through the add a phone sequence on the car system. i press the hands free entry on the iphone/settings/bluetoosh screen after it shows up and a keyboard pops up to enter the pass code. have done it several times.

a girl at the AT&T store made a decent point...she said the IPHONE has a new version of bluetooth built in (V2) and since I have a 2006 model maybe the BT installed was a older version...where as the 07 GS's have all the upgrades and maybe that's why I am the only one having issues.
Does this sound right? I may be SOL...
Any Lexus techs know of anyway to update that software for BT if there is such a thing?

Does anyone have problems with it connecting to the car when you dont want it to?? Like if my car is running or on and I am in the house or around it, it will connect to the car when making a call. So I am saying hello hello and no one is there, until I change it back to the phone connection. It has happen numerous times but only when the car is on.

I have an 06 gs430 which is synced with a 3g as well. Everything works great. I have that same deal about making outgoing phone calls. No ringing just a straight answer if they pick up.. Kinda annoying but figures it's alright.
