On mine, the first time I drained and filled 4 times. But now I just drain and fill 1 time every year.

261k miles it shifts perfectly.

It was good warm, but NOT good cold, and the colder it got the worse

Check it cold, colder the better.

Do you need to warm it up at all when checking fluid cold?

No, that is the problem. It MUST be checked COLD (and read on the COLD part of the dipstick). Maybe not all years had a cold line, my early 98 GS4 does. If the level is ok warm, and not ok cold, it will lead to the pump sucking the pan low and have the cold issue where the car goes low on fluid and my description is "kicking off into neutral".
