Sounds like a pebble in right rear fender well.
When I accelerate and get to about 45-50 and start to ease up on accalerator I get a noise that sounds just like my right tire kicked up a pebble into my fender well.
I always look in rear view mirror thinking I will see a rock bouncing in the road. I never see a rock. It only happens in RR tire well and usually from a cold start just as I get up to the 40-50 MPH range.
It happens about once every 3 or 4 times I drive my car. It has happened so much that now as soon as it happens my wife and I just look at each other and shake our heads.
What could it be?
I am taking my car for service to have a look at it tomorrow. I have 49,920 miles on it right now. Any help would be greatly appreciated.
I'm also thinking about extending my warranty to 7years/100K miles.
I also have spots on the chrome trim on the passenger and drivers door. MyLexus dealer says that is from normal wear and tear and not covered under warranty. I bought it at 43K miles so I don't know when the chrome trim began scratching off.

Can't help you with the pebble but I and I believe some other club members have had the interior door trim replaced under warranty for just the condition you are describing. Give it another try.

Look under the car in front of the wheel well. There's a little "mud guard" under there that is constantly coming loose on my car. It only takes 30 seconds to screw it back in but it never holds. I keep forgetting to clean off that spot and just glue it back into place.
