Default Chance to get 2007 ES 350 with 78k miles

I saw it advertised by a local non-Lexus, used car dealership.

The miles are definitely high, it is advertised for just under $19k. I would want to get it for a bit less than that though due to high miles. Has Nav and color combo I want.

What kind of problems should be on the look out for with with these many miles? I've heard about the transmission problem - anyway to watch out for it? Should I get a mechanic to look at it?

You have to be kidding, right? You should never EVER buy a used car of any brand without having an independent mechanic AND body shop (looking for signs of repaint and frame damage, no matter what CarFax says) look over the car from bumper to bumper. That $100-200 could save you $1000's later on.
