Codes P0456, P2238, P2241 on 2003 Lexus ES 300

Codes P0456, P2238, P2241 on 2003 Lexus ES 300
Hi all,
My engine light came on recently and I'm getting three codes:
P0456 - EVAP system very small leak detected
P2238 - Air Fuel sensor pumping current circuit low 11
P2241 - Air Fuel sensor pumping current circuit low 21
Anyone ever run into these three codes together? I've done some research into the air fuel sensors and it seems like they are easy to replace (except the cost!). One of the probably causes for P0456 is fuel cap. Could these three be related? Can I fix one and have it take care of the others, and if so, which do I try first?
I was thinking about replacing the fuel cap first, just to see what happens, but if there's little possibility that will help then I won't bother.
Any help would be appreciated!!

Hi all,
My 2003 ES300 with 280K miles has the CHECK light on. My OBDII/DTC told me that I had P0420-Catalyst system Efficiency Below Threshold (Bank 1).
So I did the followings:
1) Used my heat gun to check the temperature on my Catalytic converter before (~350-400F) and after (~400-450F), which indicates the CAT is working.
2) Replaced all my spark plugs.
3) Disconnected my battery connections, cleaned all 4 sensors' connectors and changed all of them with the original parts including Bank 1 A/F Senor 1 (PN#:89467-33060), Bank 1 O2 Sensor 2 (PN#:89465-33260), Bank 2 A/F Senor 1 (PN#:89467-33050), and Bank 2 O2 Sensor 2 (PN#:89465-33250). Now, I have P2238 Oxygen Sensor Pumping Current Circuit Low (for A/F sensor) (Bank 1 Sensor and P2241 Oxygen Sensor Pumping Current Circuit Low (for A/F sensor) (Bank 2 Sensor 1) on my OBDII/DTC. So I googled the web, someone said there is some short circuit in my sensors but what should I do?
4) Checked my Fuse #2 A/F, which is okay because it's not open circuit.

Man I am struggling with these 2 codes too. 2238 and 2241. I have NO IDEA what to look at, and I have been a tech for over a decade. These are tricky codes. It says O2 sensor, but in reality it is probably a misfire (at least in my case).

Thanks again ArmyofOne, Like I mentioned before, I have already replaced all my spark plugs. Unless I have a bad spark plug wire triggered this 2238 caused by misfire. Actually, I only one 2238 for my Bank 1 A/F Senor 1 (PN#:89467-33060) now because when I put my old Bank 2 A/F Senor 1 (PN#:89467-33050) back, it clears the 2241 code. When I have P0420-Catalyst system Efficiency Below Threshold (Bank 1), I thought my sensors were too old anyway, so I bought four new sensors with the same Toyota part#s to replace them. It turns out if things not broken don't touch it always right. I should ONLY replace Bank 1 A/F Sensor 1 not the other three sensors. It looks like I have a bad brand new A/F sensor, which I don't want to spend extra money to buy another sensor to try it again.

I changed the MAF on friday, and cleared the codes. I have since put 200 miles on the car, they have not come back, and I noticed an immediate improvement in drivability. Be that as it may, I am still going to replace the rest of the parts I ordered because even if they aren't bad yet, they are old and should be replaced. The plugs were last done at 89k when the timing belt was done, the PCV Valve has never been done, the valve cover gaskets were done at 40k, and the other stuff to my knowlege has never been touched, and the car has 161,000 miles on it as of yesterday.
