Default Kia recalls 2014 Cadenza over wheel fractures


Manufacturer: Kia Motors Corporation

SUMMARY:
Kia Motors Corporation (Kia) is recalling certain model year 2014 Cadenza vehicles manufactured February 1, 2013, through August 27, 2013, and equipped with 19" non-chrome aluminum wheels. The wheels on the affected vehicles may fracture from impacts such as large, deep potholes.

CONSEQUENCE:
A wheel fracture could cause the circular rim to separate, resulting in a loss of tire pressure, increasing the risk of a crash.

REMEDY:
Kia will notify owners, and dealers will replace the wheels with new wheels, free of charge. The recall is expected to begin in June 2014. Owners may contact Kia customer service at 1-800-333-4542. Kia's number for this recall is SC107.

Default NHTSA examining data on exhaust leak in 2011-2014 Ford Explorer


WASHINGTON (Bloomberg) -- The National Highway Traffic Safety Administration is looking into complaints that exhaust may be building up inside the cabins of Ford Motor Co.'s 2011-2014 Explorer SUVs.

NHTSA is aware of complaints that “owners are experiencing exhaust fumes or strange oders in the cabin,” spokesman Jose Alberto Ucles said in an e-mailed statement. “The agency is reviewing all available data and will take appropriate action as warranted.”

The agency stopped short of calling its effort a defect investigation, and no formal probe is listed on its Web site.

“We are confident in our current methods for quickly identifying and addressing potential vehicle issues,” Ford spokeswoman Kristina Adamski said in an e-mail. “Our decisions are driven by the data available. When the data indicates a safety recall is needed, we move quickly on behalf of our customers.”

Earlier this week, NHTSA closed a 2-year-old probe into power steering glitches in late-model Ford Explorers, signaling that they are satisfied with the automaker’s recent decision to recall the SUV. Ford said May 29 it would recall 195,527 Explorers from the 2011-13 model years as part of a sweeping campaign that included nearly 1.39 million vehicles. The National Highway Traffic Safety Administration closed its “preliminary evaluation” of the Explorer on June 12, a document posted June 16 on the agency’s Web site said

With emission levels so low today with even the highest-polluting gasoline engines, seems like it would take a pretty serious leak (say, for example, through a bad exhaust-pipe joint or seal underhood), for enough CO to get sucked into the climate-control air-intake system to be a real threat to the driver or passengers inside.

Statement: Regional Field Action

June 23, 2014 , Auburn Hills, Mich. - Chrysler Group has agreed, in principle, to honor a National Highway Traffic Safety Administration (NHTSA) request to replace airbag inflators in certain vehicles registered in four U.S. regions.

This is not a safety recall. Chrysler Group has not identified a defect. This is a field action conducted out of an abundance of caution.

The action, expected to receive final approval this week in accordance with Chrysler Group protocol, follows consultation with NHTSA and other automobile manufacturers that use the same airbag inflator.

The Company is aware of a single related injury involving one of its vehicles. The vehicle was equipped with the component, currently under investigation by NHTSA.

The regions affected by Chrysler Group's field action are Florida, Hawaii, Puerto Rico and U.S. Virgin Islands.

Customers will be advised when they may schedule service. Those with questions or concerns may call Chrysler Group's Customer Information Center at 1-800-853-1403.

Statement by American Honda Regarding Front Airbag Inflator Market Actions
06/23/2014 - TORRANCE, Calif.
Honda will voluntarily recall certain vehicles in the United States in three separate market actions to replace one or both front driver's and passenger's airbag inflators, free of charge. Two of the market actions are specific to states and U.S. territories with high absolute humidity. It is possible that front airbag inflators in affected vehicles may deploy with too much pressure, which may cause the inflator casing to rupture and could result in injury. Honda is aware of one claimed injury involving a driver's front airbag. Honda is not aware of any other injuries or fatalities related to these market actions. While the specific number of affected vehicles has not yet been determined, Honda estimates that the three actions will include a total of more than two million vehicles in the United States.

In two of the market actions, Honda is working in cooperation with the manufacturer of the airbag inflators and NHTSA to recall certain vehicles in 10 U.S. states and territories (Alabama, Florida, Georgia, Hawaii, Louisiana, Mississippi, South Carolina, Texas, Puerto Rico and the US Virgin Islands) based on the high absolute humidity levels common to those areas. While the states and territories in which Honda is conducting these actions are broader than those suggested by the manufacturer of the airbag inflators (Florida, Hawaii, Puerto Rico and the US Virgin Islands), Honda has chosen to expand the affected area for these market actions because of similar environmental conditions in those areas.

Nissan Statement

"Nissan is announcing the following field actions concerning Takata front passenger air bags on certain Nissan and Infiniti vehicles. These actions supplement the 2013 recall to replace Takata front passenger air bag inflators in certain Nissan and Infiniti vehicles.

1. Based on new information received from Takata in June 2014, Nissan is adding vehicles that were not included in the 2013 global recall campaign. Certain MY2001 to MY2003 Nissan and Infiniti vehicles are affected by this recall campaign.
2. Nissan will initiate a regional field action in Puerto Rico, Florida, Hawaii, and the Virgin Islands following new information received from Takata and recent consultations with the National Highway Traffic Safety Administration (NHTSA), based on the high levels of absolute humidity in those areas to replace front passenger air bag inflators manufactured between January 1, 2003 and July 31, 2004. The air bags in this date range were not subject to the Takata Front Passenger Air Bag Global Recall Campaign. MAZDA STATEMENT ON SPECIAL SERVICE PROGRAM FOR CERTAIN MAZDA6, RX-8, MAZDASPEED6, AND MPV VEHICLES WITH AIR BAG INFLATORS SUPPLIED BY TAKATA CORPORATION AND TK HOLDINGS, INC.


WASHINGTON, DC (June 23, 2014) – Mazda North American Operations has notified NHTSA that it plans to voluntarily implement a Special Service Program in limited regions of the United States as a result of several field reports regarding airbag deployments for a variety of vehicle brands equipped with air bag inflators supplied by Takata Corporation and TK Holdings, Inc. On certain vehicles, the air bag may not deploy properly in the event of an accident.

Approximately 34,600 vehicles that were originally sold or are currently registered in Florida, Hawaii, and Puerto Rico are affected.

There is insufficient information at this time to determine the cause of these incidents or if they are related to a safety defect, but Mazda is working with the supplier and NHTSA to investigate these incidents as quickly as possible.

Mazda has determined that the subject vehicles of the SSP are certain model years of the Mazda6, Mazdaspeed6, RX-8, and MPV, which are equipped with similar types of airbag inflators.

Specifically the vehicles are identified as:

2003-2007 MY Mazda6 built from October 4, 2002, through July 31, 2007
2004-2008 MY RX-8 built from April 23, 2003, through July 26, 2007
2006-2007 MY Mazdaspeed6 built from August 25, 2005, through June 28, 2007
2004 MY MPV built from August 21, 2003, through July 28, 2004

Mazda has received one report of a driver injury during airbag deployment.

Owners of record will be notified of this issue and instructed to take their vehicles to a Mazda dealer to replace the air bag inflator with a new one. The work will be performed by Mazda authorized dealers, free of charge to the vehicle owners.

The expected dates for mailing owner notification letters and providing notification to dealers are currently undecided. It will depend on the availability of replacement parts from the supplier.

WASHINGTON, DC (June 23, 2014) – Mazda North American Operations has notified NHTSA that it plans to voluntarily implement a Special Service Program in limited regions of the United States as a result of several field reports regarding airbag deployments for a variety of vehicle brands equipped with air bag inflators supplied by Takata Corporation and TK Holdings, Inc. On certain vehicles, the air bag may not deploy properly in the event of an accident.

Approximately 34,600 vehicles that were originally sold or are currently registered in Florida, Hawaii, and Puerto Rico are affected.

There is insufficient information at this time to determine the cause of these incidents or if they are related to a safety defect, but Mazda is working with the supplier and NHTSA to investigate these incidents as quickly as possible.

Mazda has determined that the subject vehicles of the SSP are certain model years of the Mazda6, Mazdaspeed6, RX-8, and MPV, which are equipped with similar types of airbag inflators.

Specifically the vehicles are identified as:

2003-2007 MY Mazda6 built from October 4, 2002, through July 31, 2007
2004-2008 MY RX-8 built from April 23, 2003, through July 26, 2007
2006-2007 MY Mazdaspeed6 built from August 25, 2005, through June 28, 2007
2004 MY MPV built from August 21, 2003, through July 28, 2004

Mazda has received one report of a driver injury during airbag deployment.

Owners of record will be notified of this issue and instructed to take their vehicles to a Mazda dealer to replace the air bag inflator with a new one. The work will be performed by Mazda authorized dealers, free of charge to the vehicle owners.

The expected dates for mailing owner notification letters and providing notification to dealers are currently undecided. It will depend on the availability of replacement parts from the supplier.

Default Ford recalling nearly 5,700 2014 F-150s for power steering issue


Ford is recalling 5,675 of its 2014 F-150 pickups over an issue with the electric power steering system. There haven't been any injuries or accidents due to the issue.

Ford spokesman Mike Levine told Autoblog that the "electric power-assisted steering and motor position sensor gear were incorrectly installed by a supplier," leading to the recall.

While the recall is larger on the surface, it's important to note that only 260 trucks have been delivered to customers. The remaining pickups are currently en route to dealers. The affected vehicles were manufactured between May 26 and June 19.

Owners are being asked to park their trucks until the issue has been resolved.

DETROIT -- General Motors is calling back 392,459 of its newest pickups and SUVs in the U.S. for a problem that could allow the trucks to slip into neutral unexpectedly, as the automaker continues a sweeping safety review in the wake of its bungled handling of a faulty ignition switch.

GM said in a statement today that it is recalling four-wheel-drive versions of model year 2014-2015 Chevy Silverado and GMC Sierra pickups, and 2015 Suburban, Tahoe, Yukon and Yukon XL SUVs in the United States.

GM said the trucks' transfer case "may electronically switch to neutral without input from the driver." If the problem happens while the truck is in motion, "the customer would experience loss of motive power," GM said in a separate notice to dealers.

If the truck is stopped or parked, it could roll away if the parking brake isn't set. GM said it isn't aware of related crashes or injuries.

Dealers will reprogram the transfer case control module, GM said.

GM also disclosed two smaller recalls: 4,794 Chevy Caprice police cars and SS sports sedans to repair a problem with the windshield wiper module assembly that could cause the wipers to stop working; and 1,919 units of the 2014 Corvette with the FE1 or FE3 suspension to repair a possible "insufficient weld in the rear shocks that could lead to a fracture an/or reduce the shocks' service life," GM said.

The recalls raise GM's total count for the year to 48 callbacks, covering about 20.5 million vehicles globally. Some of those are counted more than once because they've been called back for multiple recall-related repairs.

GM CEO Mary Barra reiterated in a TV interview this week that more recalls were likely. GM officials have said they are accelerating their review of older, lingering problems while more closely scrutinizing potential trouble with new vehicles.

GM Announces Four Vehicle Recalls

DETROIT – General Motors (GM: NYSE) today announced four safety recalls covering 428,211 vehicles in the United States.

Dealers will replace the driver's side air bag inflator in 29,019 2013-2014 model year Chevrolet Cruze sedans in the U.S. and 4,066 in Canada. The inflator in the driver's front air bag may rupture and/or the air bag may not inflate during air bag deployment. A rupture could propel metal pieces of the inflator into the vehicle cabin, possibly striking and seriously injuring the driver or other vehicle occupants. Additionally, if the inflator does not inflate the air bag, there is an increased risk of injury to the driver. GM is aware of one injury related to the issue.

Dealers will recalibrate transfer case control module software in 392,459 full-size pickup trucks and SUVs in the U.S. and 53,607 in Canada. An additional 20,874 were sold outside the U.S. and Canada. Affected are four-wheel-drive versions of the 2014-2015 Chevrolet Silverado and GMC Sierra; the 2015 Chevrolet Tahoe and Suburban; and the 2015 GMC Yukon and Yukon XL.

In these vehicles, the transfer case may electronically switch to neutral without input from the driver. If this occurs while a vehicle is in motion, no power will go to the wheels. GM is not aware of any crashes or injuries related to this issue.
Dealers will replace the two rear shock absorbers in 1,939 2014 model year Chevrolet Corvettes in the U.S. with the FE1 or FE3 suspension to repair a possible insufficient weld in the rear shocks that could lead to a fracture and/or reduce the shocks' service life. Thirty-three additional vehicles were sold in Canada and another 82 were exported. GM is not aware of any crashes or injuries related to this issue.

Action: A Preliminary Evaluation has been opened.
Summary: The Office of Defects Investigation (ODI) has received four complaints alleging that the tunnel carpet cover trim panel on Model Year (MY) 2012 through 2014 Nissan Versa, Versa Sedan and Versa Note vehicles has trapped the edge of the driver's shoe. The alleged condition interfered with the driver's ability to reduce the rate of acceleration as desired and also interfered with the driver's ability to apply the brakes when needed.

One complainant, VOQ 10597004, alleges that their foot was trapped by the trim panel such that they had to use their right hand to grab their leg and pull up to get their foot free, almost causing a crash during the event. The trim panel in question (Figure 1) is located next to the accelerator pedal and the lower corner is positioned such that it is near the right outer edge of the driver's shoe. The complainant supplied a photograph (Figure 2) showing how the edge of the panel wedged itself in the driver's shoe. Complainant 10573057 alleged that when attempting to remove their foot from the accelerator pedal to apply the brake, their shoe was caught by the trim panel which delayed braking ability several times. Two of the four complainants note that the vehicles were rental cars.

A Preliminary Evaluation has been opened to asses the scope, frequency and safety-related consequence of the alleged defect.

The VOQs associated with this investigation are: 10597004, 10573057, 10503811, and 10498000. A fifth VOQ (10578373) is from a foreign country and is not counted in this resume.

Default GM recalling 8.4M cars, 8.2M related to ignition problems


General Motors today announced a truly massive recall covering some 8.4 million vehicles in North America. Most significantly, 8.2 million examples of the affected vehicles are being called back due to "unintended ignition key rotation," though GM spokesperson Alan Adler tells Autoblog that this issue is not like the infamous Chevy Cobalt ignition switch fiasco.

For the sake of perspective, translated to US population, this total recall figure would equal a car for each resident of New Hampshire, Rhode Island, Montana, Delaware, South Dakota, Alaska, North Dakota, the District of Columbia, Vermont and Wyoming. Combined. Here's how it all breaks down:

The Cadillac models in the second grouping* have been found to be subject to unintended key rotation because of accidental bumping of one's key fobs (a condition similar to the June 13 Camaro recall), whereas the other, larger group of older vehicles is not subject to bumping. In either case, this is not like the infamous Chevy Cobalt ignition switch issue – the switch in these vehicles meet GM specifications, the company says the problem is with the key design itself.

The solution won't be a new ignition switch at all, it will be sending additional detachable key rings to owners (allowing drivers to disconnect their car key from the rest of their key chain to reduce strain on the system) and/or inserts designed to fit within the ignition key head itself to alter the keyring attachment loop from a slot to a hole (because the slot can cause the weight of the keychain to hang asymmetrically, thereby increasing the amount of torque on the key and as a result, the switch.

Company spokesperson Alan Adler, tells Autoblog that GM "changed the [key] design in late December of 2010," so vehicles built after that time are not affected by the issue.

GM is aware of seven crashes, eight injuries and three fatalities related to these issues. "The fatal crashes occurred in older model fullsize sedans being recalled for inadvertent ignition key rotation. There is no conclusive evidence that the defect condition caused those crashes," GM said in a press release

GM has made changes to every process that affects the safety of its vehicles, and the company has acted or will act on all 90 of the recommendations put forward by former U.S. Attorney Anton Valukas in his independent report to the company's Board of Directors.

GM expects to take a charge of up to approximately $1.2 billion in the second quarter for the cost of recall-related repairs announced in the quarter. This amount includes a previously disclosed $700 million charge for recalls already announced during the quarter.

Until the ignition recall repairs have been performed, it is very important that customers remove all items from their key ring, leaving only the vehicle key, and always use their seat belts. The key fob, if present, should also be removed from the key ring.

We may (?) or may not see a Jeep Grand Cherokee recall or TSB (Technical Service Bulletin) over this, but something not long ago occurred down the street from my house, where I buy my Shell gas, that might (?) prompt one.

Here is a brief report of the crash from the local police: It has since been revealed that the runaway vehicle was a Jeep Grand Cherokee (didn't say what year). I did not see what happened (or the runaway Jeep), as I was away most of that day, and, when I got back, the police and fire units had the area cordoned off.

This was quite a runaway. The Jeep managed to run over three 6-inch high concrete curbs, hit three vehicles, and push the last one into the gas pump.



Arrest – Felony Hit and Run/Reckless Driving 14-04731; Maple Avenue near James Madison Drive, West; April 27 9:56 a.m. The driver of the first vehicle, an employee of Flagship Cars Wash, was attempting to exit the wash tunnel in a customer’s vehicle when he stated the vehicle accelerated unexpectedly. It traveled over the curb, entering Maple Avenue, ran over the median, entering the west bound lanes of Maple Avenue and collided with a second vehicle. After striking the second vehicle the vehicle continued across the west bound lanes of Maple Avenue, entered onto the James Madison Shell Station parking lot and collided with a third vehicle. The collision with the third vehicle occurred with such force that vehicle completely took out a gas pump.
The driver of the second vehicle became trapped in his vehicle which required the Fairfax County Fire and Rescue Department to cut the driver’s door off the vehicle in order to extricate the driver. The driver was then transported to an area hospital for injuries suffered in the collision.

The employee fled the scene prior to the arrival of the police however he was located a short time later at the rear of the James Madison Shell. PFC Vazquez arrested a 53-year-old man of Glengyle Drive in Vienna for Felony Hit and Run.

He was transported to the Fairfax County Adult Detention Center where he was charged with Felony Hit and Run and Reckless Driving. He was held on a $2,500.00 bond.

In addition to the criminal charges filed against the driver, the car wash and its insurance company, of course, is liable for for damages, but, from what I've been told, they have, in turn, have sued Chrysler, claiming that the Jeep had runaway acceleration.


It's also lucky, of course, that there was no major fire or even an explosion at the gas station....my favorite one, where I buy most of my gas. An explosion would have also taken out some businesses and apartments right next to the station, and hurt or killed many people. The station has a large shut-off emergency **** that immediately shuts all the pumps down in case of an emergency.

The gas station had to wait almost two months to get a new pump installed and operational because the destroyed pump was an older electro-mechanical pump that is no longer built. (there are 7 more pumps at the station, so not that much business was lost). They had to order a new digital pump and wait to have it installed next to the older pumps, where the old one had been.

Additionally 20,134 North American vehicles are being recalled because "insulation on the engine block heater power cord (if equipped) may become damaged during very cold conditions." 2,990 of these vehicles are in the United States.

As part of our continuous product testing and improvement process, Graco identified that some of the harness buckles used on our infant and toddler car seats sold in the United States are difficult to open. In response, we are recalling the harness buckles used on select infant, toddler convertible car seats and harnessed booster seats.

There have been no reported injuries related to this issue or the safe use of the car seats. Graco will continue to offer a free replacement buckle to any consumer who has a toddler car seat or infant car seat not included in this recall, but would like to update their buckle to Graco's current buckle design.

Consumers can determine if their infant and toddler car seats are affected and order a replacement harness buckle using our online order form. While waiting for a replacement kit, it is safe to use the infant and toddler car seat. If the buckle becomes sticky or difficult to open, Graco recommends cleaning it by following the cleaning instructions as this will definitely make the buckle easier to use. Q: What is the difference between the old and new harness buckle?
As early as January 2013, Graco began equipping our infant car seats with a new and improved buckle. While some of the buckles look the same, the updated buckle has an improved design to make it easier to use and has been rigorously tested. This recall impacts only the older model of this buckle, used in car seats manufactured prior to May, 2013. The updated buckle (car seats manufactured after May 2013) is not being recalled, as it provides significantly improved usability and performance which are unique to infant car seats:
While it looks the same on the outside, two thirds of the internal mechanism has been updated, making it easier to use on infant seats.
The updated buckle is easier for parents to tilt and release the tongues from the buckle due to the reclined position of the infant seat and the improved ergonomic design of the buckle's button.
The size and shape of the buckle is more appropriate for infant seats and smaller occupants, including where the buckle rests on the child and the pressure exerted on the buckle when it is being used by a parent.
The redesign and the location of the buckle in relation to the child's hands and mouth on infant seats reduces the likelihood of the buckle becoming sticky from food and juice, which can make it difficult to open and close.
The improved design reduces the friction in the internal components, which reduces the likelihood of it becoming sticky if kids spill food and drink on it.

Statement: Expansion of Wireless Ignition Node (WIN) safety recall

July 1, 2014 , Auburn Hills, Mich. - Out of an abundance of caution, Chrysler Group is expanding a previous safety recall to install a more robust WIN module detent ring.

A previous recall in 2010 included 196,000 Chrysler Town & Country, Dodge Grand Caravan and Dodge Journey vehicles built August 2, 2009 through June 17, 2010.

The expansion includes an additional 695,957 vehicles manufactured January 29, 2007 through August 1, 2009.

Vehicles equipped with a FOB key may experience inadvertent ignition switch displacement from the "run" to "accessory" position while driving. If the switch is not fully engaged in the "on" position of the detent ring inside the module, the spring-loaded switch may allow the key to spring past the "on" position and into the "accessory" position. This action will shut off the engine and passive restraint systems, including air bags.

The issue was originally identified through the Company's internal quality control research and customer input. Chrysler Group is unaware of any injuries related to the expansion.

Chrysler Group dealers will, at no cost to customers, install the new WIN detent ring in affected vehicles.

Chrysler Group will contact affected customers and advise them when service is available.

An estimated 525,206 vehicles are located in the U.S.; 102,892 are in Canada; 25,591 are in Mexico and 42,268 are outside the NAFTA region.

SUMMARY:
This defect can affect the safe operation of the airbag system. Until this recall is performed, customers should remove all items from their key rings, leaving only the ignition key. The key fob (if applicable), should also be removed from the key ring. Chrysler Group LLC (Chrysler) is recalling certain model year 2009-2010 Dodge Journey vehicles manufactured June 29, 2007, to June 17, 2010, and 2008-2010 Dodge Grand Caravan and Chrysler Town and Country vehicles manufactured January 26, 2007, to June 17, 2010. Road conditions or some other jarring event may cause the ignition switch to move out of the run position, turning off the engine.

CONSEQUENCE:
If the ignition key inadvertently moves into the OFF or ACCESSORY position, the engine will turn off, which will then depower various key safety systems including but not limited to air bags, power steering, and power braking. Loss of functionality of these systems may increase the risk of crash and/or increase the risk of injury in the event of a crash.

REMEDY:
Chrysler will notify owners, and dealers will modify the ignition switch, free of charge. The manufacturer has not yet provided a notification schedule. Owners may contact Chrysler customer service at 1-800-853-1403. Chrysler's number for this recall is L25. Note: This is an expansion of recall 11V-139.
