I know every couple of years we have an oversized rig get wedged under one of the bridges in downtown Nashville. Regular trucks pass just fine, its the oversized guys that should use the I440 loop instead of the downtown loop that get into trouble. Major PITA because usually they have to shut that section of road down(and traffic on the bridge is diverted) for inspection and repairs once they get the wreck cleared.

Out of Warranty
Oilfield or heavy construction equipment loaded on flatbed or lo-boy trailers is often the cause of local crashes at bridges or tunnels because the crew just loads up the truck and drives off - without checking the overall height. One of my old clients had a heavy steel I-beam bridging the driveway out of his yards onto the highway. It was set at 15' - a foot lower than the max (unpermitted) height allowed on the nation's interstates. If your load was too tall, it wouldn't make it out of the yard. If you're going to tear up your equipment, it was a better idea to do it on your own property - where at least you had the tools and manpower to unload the truck and do the repairs.
