The E-ticket ride in Naples use to be the bus ride from Capadimonte down to the Bay, passing through these narrow streets. For three lire (roughly 2/3¢ US), you could ride through streets that were no more than a foot or two wider than the bus. Many parts of the route traverse two-way streets, and all include at least one narrow sidewalk that when needed, becomes part of the roadway. Yeah, the major vehicle control functions were accomplished by use of the horn. 
The locals saw nothing especially dangerous in that trip, but American and German tourists were terrified for the first few trips. We tend to be a little more regimented in our traffic laws - they're just nuts.
