Sirius antenna TSB - new tape kit

Does anyone have the TSB or the installation instructions for the PTS31-00051-TK retofit kit to fix the Sirius SIGMA antennas that are falling off the windshields? I received the kit from my dealer to fix my antenna, but I am not sure exactly what I am supposed to do.

Ya mine antenna went bad already and had it replaced then its fell off the windshield twice already, I just made the dealer razor blade off all the old tape and replace it, they thought it would be nice to put the new tape over the old tape and thats what caused it to fall off. I have the one thats on the front windshield, I think now that I want one that goes on the rear deck behind the backseats. I wonder which antenna is the best.
